Changes to the SIQ
Our revised Site Questionnaire has changed slightly. Most of the changes are to accommodate submissions from Canadian sites. We have introduced some notable other changes, too:
– Questions 1 and 4 include a description of the Quorum Web Portal. This is to introduce our portal to sites and to provide the PI or Site Coordinator the opportunity to opt out of being assigned a portal account.
– Date for audit information (Question 6b): Requesting dates of any reported audit information helps our staff determine whether the Board has reviewed information previously. Asking for the date also helps our staff identify audit reports that are older than three years, as the Board no longer reviews older audit reports.
– Emergency Measures (Question 11): We have rephrased the question to clarify our Board members’ expectations regarding how a site will handle any emergency situations involving participants. A site that does not agree to these expectations should provide a written explanation as part of its submission.
