Updated Central Study Questionnaire
Quorum Review IRB has re-engineered our Central Study Questionnaire. We use the Central Study Questionnaire to collect information about the protocol upon submission. Our revised version requests sponsors and CROs to provide similar information as the previous version of the form, but the questions have been re-ordered and re-worded for clarity. Quorum will continue to accept previous versions of the form through the end of 2010.
The corresponding versions of the forms for single sites and principal investigator generated studies, the Single Site Study Questionnaire and the Principal Investigator Generated Study Questionnaire, have been updated in a similar fashion.
